Taking our tests is a simple and straightforward procedure.

Firstly, order a test kit. You will receive a sampling kit containing the following:

	1 Instruction leaflet (and an allergy questionnaire     for a test)
	2 lancets to prick finger (1 spare provided)
	1 blood sample collection tube
	1 safety transport tube and lid
	1 antiseptic wipe
	1 waste bag
	1 return box container
	1 Pre-addressed (freepost) envelope

The allergy questionnaire form consists of questions related to your food problem symptoms, medical background, family allergy history and allergy test history.

Why do we need this information? Only a proportion of people who respond to a food with high IgE antibodies will be affected by allergy symptoms. The data you provide in our allergy questionnaire allows us to gauge with more accuracy whether it is worth pursuing dietary changes for the foods that you show a higher than average response to.

All information that you provide will be kept strictly confidential, and anonymous.
